Conditions not working with Umbraco forms?

In short: Using Umbraco 10.6.1 and Umbraco Forms 10.4.0 I followed the setup instructions on Umbraco docs, Forms works as expected apart from the conditions. Conditions don't seem to work at all. No errors, nothing. I was hoping somebody has had similar issues and most importantly: a solution.
